Providing support in the local community

The Outreach Service provides support and activities in the local community or in the home to children or young adults with severe learning difficulties, physical disabilities, and/or autism.

A Coordinator works in partnership with the young person, their family to ensure we are able to provide the most appropriate support. The service is available 7 days a week.

We can help in a number of ways:

  • Provision of one-to-one quality time with young people at home or in the community.
  • Support for young people to attend youth clubs, groups or play schemes.
  • Help for young adults to experience activities such as the cinema, bowling, or a night out with friends.
  • Support for young people to attend appointments or complete outings for example shopping trips.
  • Provision of a few hours respite for parents/carers to spend on their own or with siblings.
  • Provision of personal care.

Our Coordinators carefully match children to trained support workers. We are able to provide a variety of opportunities, with activities chosen by the child whilst being supported by a friendly face.

A Coordinator will come and visit you to discuss the service and agree a care plan that will be personal to you and outline how we can meet your needs.

There are number of routes that you can receive funding for these services.

You can use your Southampton ONE2ONE vouchers, Hampshire Gateway number, Social Services funding or private payment.
